Referrals and Scripts Requests
Please make an appointment with your Doctor to discuss any requirement to see a Specialist and to arrange a referral. We ask that you obtain your scripts during consultation. Repeat scripts require an appointment.
Medical Certificates
Please advise at consultation if this is required as the law prohibits backdating of certificates.
Test Results
These require an appointment with your Doctor. Your Doctor will advise when they expect the results to arrive at the Practice. Any Patient who has URGENT abnormal results will be contacted to see their Doctor. If unable to be contacted by phone, a letter will be sent.
Reminder Systems and Recalls
In order to provide patients optimal care, we have a reminder system in place as well as participating in State/Federal Reminder Registers. Patients are notified by phone or mail when due for vaccinations, pap smears, review check-ups, health assessments, etc. Please inform your Doctor if you do not wish to participate.
Home Visits
Our practice does not provide home visits as a routine service. However, should a practitioner deem it necessary, arrangements for home visits can be made at their discretion. Please contact reception for further information or to request to speak to a doctor regarding home visits.
Medical Records
These are the property of the practitioner. However you are entitled to access or obtain copies when completing relevant paperwork. A patients summary/records will be provided to other practices when signed "Request for Transfer" is completed. A small fee might apply. Your medical record is a confidential document. It is our Practice's policy to ensure that the information is only available to authorised members of staff. We abide by the ten National Privacy Principles at all times.
Self Identification
Helping close the gap with one simple question: are you Aboriginal or Torres Strait Islander origin? Why? Because Governments need good quality information to measure the health of the Australian population and to support the effective, efficient health services of All Australians. This question is asked of ALL patients regardless of your appearance, country of birth, or whether they are personally known to the health professional.
Telephone Calls
Doctors do not routinely take phone calls. Our Practice nurse will help you decide whether the matter requires an appointment and/or other medical assistance and will facilitate the message to your Doctor.
Patient Feedback and Concerns
Your comments on our medical services are always welcome. Any difficulties can be discussed with your Doctor or Practice Manager. Complaint forms and suggestions are available from Reception. A "Privacy Laws Protect you" Poster has information regarding complaints with Health Services Commissioner (1300 582 113)/ National Privacy Commissioner (1300 666 444).

If this service is required, please let us know when booking an appointment and this can be arranged through the Translating and Interpreting Service (TIS).
